The VNA and Hospice of Greater Milford, a department of Milford Regional, provides a dignified and compassionate alternative to institutionalized care by offering professional and supportive health services in your home. This venerable organization, which has served the home health needs of individuals for over 90 years, recently completed the full continuum of care by adding hospice services.
The combination of these services can prevent premature placement of an individual in an acute care hospital or a chronic care setting, allow for an earlier discharge from the hospital or nursing home, or provide patients with added comfort and support during their final days.

Nursing
Our nursing staff comprises licensed professional clinicians who provide a wide range of care to meet the diverse needs of our patients in the home setting. As well as providing direct patient care, our nurses are skilled in observation and assessment, case management and patient education. A plan of care is established, working closely with the patient, physicians and other community providers, to meet each individual need. An interdisciplinary team approach is utilized and each patient's progress is measured and evaluated. The VNA provides the following types of nursing care:
Our nurses provide patient and family education about disease management, along with quality nursing care and treatment in the home-care setting. These services include medication instruction, lifestyle modification and other skilled nursing interventions to meet our patients' needs.
Prenatal, postpartum, neonatal, infant and early childhood services are provided by highly trained and experienced nurses with current knowledge of women, infant and children's health issues.
Our Mental Health nurses provide a specialized program for patients with primary psychiatric disorders or secondary emotional problems who are having difficulty coping and are in need of in-home care. The population served ranges from young adults to the elderly.
Our VNA employs nurses with specialized certification in wound and Enterostomal (ET) care, providing state-of-the-art treatment of wounds and ostomies. With this new technology, wounds heal better and quicker.
The VNA provides advanced care for patients diagnosed with a terminal illness. Nutritional counseling, pain control, evaluation and monitoring, along with emotional support, are among the services provided.
Hospice is a philosophy of care for patients who are dealing with a life-limiting illness and may have six months or less to live. The goal is to enable patients to maintain a pain-free
life and manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones. Our hospice services include a team consisting of a nurse, social worker, home health aides for personal care and pastoral counseling. While our hospice service has a medical director, the patient keeps his or her own physician. An important focus of hospice care is emotional support for family and friends close to the patient. Our hospice team is well versed in the progression of emotions that accompany this important time of life and the feelings of loss that are inevitable for everyone in the circle. Even after the patient passes away, hospice continues as a touchstone for loved ones for several months. New patients can enter hospice care directly. Our traditional home health care patients with terminal illness can transition to hospice using the Bridge program approach. As patients receive palliative care, they use this time to make decisions about treatment and hospice care options, all while getting to know our hospice team.
A VNA social worker offers support and assistance to patients and their families. Services include coordination of care, mobilization of community resources, benefits information, long-term-care assessment and assistance with placement, when appropriate.
Our VNA provides a specialized rehabilitation team, comprised of registered physical, occupational and speech therapists, who work with individuals after a debilitating illness. Services include a home assessment evaluation, patient and family education to improve strength, increase mobility and assist with lifestyle adaptation for specific conditions.
Certified Home Health Aides (HHA) assist patients in their homes with bathing, dressing, exercises and meal preparation. All HHAs receive specialized and ongoing education and are supervised by our clinical staff.

Telehealth monitoring is designed to improve homebound patients’ quality of life and medical outcomes by successfully managing chronic disease. The Telehealth nurse installs a small portable monitor with easy to read instructions and clear voice prompts in the patient’s home.
Vital health measures such as blood pressure, pulse, oxygen level and weight can be taken and electronically sent to the VNA daily. The Telehealth nurse is able to monitor each patient’s condition through the VNA’s electronic medical record, identify potential problems, and notify the visiting nurse when changes are noted.
Putting patients in charge of their own care leads to an overall improvement in health and improved outcomes. It also helps them avoid acute episodes with hospitalizations, which are very costly physically, emotionally, and financially. Patients being monitored and answering questions on a daily basis are comforted knowing they are being looked after and feel less isolated.
Our VNA arranges for medical supplies and equipment needed for home care.

The VNA currently services 18 area communities (listed below). Our caregivers average more than 650 healthcare visits daily. The VNA is accredited by The Joint Commission. Medicare, Medicaid, HMOs and commercial plans include Home Health Services. A sliding fee scale is also available.
